Mental Health & Addictions Detailed Planning and Action Team Recommendation Report

The Mental Health and Addictions Detailed Planning and Action (DPA) team was formed in September 2007 and officially launched it’s activities at the first Working Group meeting in October 2007.   The 66 member team is comprised of health care leaders and professionals, community members, families, clients, LHIN staff and guided by shared leadership from two senior executives from the Mental Health and Addictions Agencies.

The DPA team has met collectively and as task teams to address our mandate of developing and integrated service delivery model for mental health and addictions for the Mississauga Halton Local Health Integrated network.  Clients and families are at the centre of our investigation and our recommendations.

The report below details the recommendation of the integrated service delivery model that was created.  This recommendation encompasses a co-location, multidisciplinary model supported by a central hub for system administration and management, with satellites centres strategically located within the Mississauga Halton LHIN.  Click on the link below to access the complete report:

Ministry/LHIN Effectiveness Review

In 2005, the Ministry of Health and Long-Term Care established 14 LHINs (local health integration networks) to increase local decision-making with respect to health care and to improve access and equity, system integration and coordination, as well as quality, sustainability and the overall health of the province. LHINs are responsible for managing their local health systems with a focus on planning, funding and integrating local health services to meet the needs of the residents within their LHIN.

As set out in the Ministry-LHIN Accountability Agreements, the Ministry and the LHINs completed an ‘effectiveness review’ conducted by third-party consultants KPMG. The purpose of the review was to evaluate the efficacy of the transition and devolution of authority from the Ministry to the LHINs and also sought input from provincial associations and a sample of health service providers.

A Call to Action!  Preventing Falls in Our Seniors Population

In 2007, through financial support from the Ministry of Health and Long-Term Care's Emergency Department Support Fund, a cross-sectoral Falls Prevention Steering Committee was established to develop a strategic framework which, when implemented, would reduce falls and falls-related injuries in seniors, ultimately reducing ED visits and hospitalizations.

Two resource guides have been developed from the project, including: a framework for falls prevention, an inventory of falls prevention efforts in Mississauga Halton, and links/references to best practices in falls prevention by sector.  Please click on the links below to access these valuable resources.

Integrated Priority Report: Local Opportunities for Health System Integration

The Local Opportunities for Health System Integration Report was prepared to summarize and record discussions that the Mississauga Halton LHIN Steering Committee held in late 2004. The report provides an excellent foundation to inform the transformational journey of the LHIN.
