Primary Care Lead (Family Physician)
Diabetes Regional Coordination Centre
Status: Part-Time – up to .5 FTE (up to 5 days in a two week period)
Site: Oakville-Trafalgar Memorial Hospital site of Halton Healthcare Services
As a part of the Ontario Diabetes Strategy, the Ministry of Health and Long Term Care (MOHLTC) is creating a Regional Coordination Centre (RCC) in each of the 14 Local Health Integration Networks (LHINs). The RCCs are responsible for the coordination of and improvements in outcomes for diabetic prevention, education and care within the province including both general and priority populations.
The RCCs will organize and coordinate regional diabetes programs and services in collaboration with community diabetes programs, primary care providers and the Community Care Access Centre. These centres will leverage local resources and existing expertise to ensure integration of diabetes services across the continuum of care, from prevention to primary and tertiary care, so that individuals living with, or at risk of developing diabetes have access to the services they need. Through the coordination of diabetes services across the Mississauga Halton LHIN, the RCC will demonstrate the region's ongoing commitment to quality care, clinical excellence and patient satisfaction.
Halton Healthcare Services (HHS) was chosen as the host-site for the Mississauga Halton LHIN.
Job Description
The Primary Care Lead will:
- Lead regional program development from a primary care perspective.
- Act a resource, leader, and advocate for primary care providers in the region.
- Work with the regional administrator to build a regional network of diabetes care providers.
- Be the primary care clinical leader within the regional organizational structure.
- Work as a member of the diabetes program leadership team to achieve all aspects of high quality diabetes care at the primary care level.
- Help address diabetes service delivery issues within the region, such as quality management and unattached patient management.
- Collaborate with a provincial network of primary care leads.
Qualifications
Halton Healthcare Services is seeking individuals who are competent in their chosen profession; patient/customer focused; effective communicators; caring and respectful; and are fiscally responsible.
- Experienced Family Physician with proven expertise and interest in diabetes
- Active member of the College of Physicians and Surgeons of Ontario
- Prior experience in a physician leader role preferred
- Leadership training, e.g. Physicians Manager Institute would be an asset
- Demonstrated ability to lead and influence change through clinical credibility
- Effective collaboration and negotiation skills
- Strategic planning experience
- Proven ability to create and sustain effective partnerships
- Superior communication skills

Advanced Practice Nurse - Lead
In keeping with the Mississauga Halton Local Health Integration Network (LHIN) vision to reduce reliance on hospitals by enabling individuals to die at home, the Mississauga Halton CCAC is seeking an Advanced Practice Nurse (APN) Lead to be involved in the program planning, implementation and evaluation of a newly developed community palliative care model for Mississauga Halton. The successful candidate will also act as team lead for any future Community Palliative APNs hired in the new fiscal year.
In addition to this leadership component, the APN component of this role will include providing comprehensive advanced nursing consultation to support the discharge of palliative individuals from our Mississauga Halton hospitals to home and conversely in the community to support complex clients remaining in their home. As part of the MH CCAC’s Community Palliative Care Program, the APN will have the opportunity to engage in all domains of advanced practice nursing with a focus of improving quality of life for community residents and their families. Additionally, the APN will provide leadership and professional development through coaching and mentoring nursing and physician colleagues, and participating in formal and informal educational initiatives.
