Primary Care Education Opportunities

Smoothing the Transitions from Hospital to Home – November 22nd 

Primary care plays a key role in supporting patients in their post-acute recovery.  Please join Dr. Michael Kates, Primary Care Lead in the Mississauga-Halton LHIN, on Thursday November 22, 2012 to learn how the Rapid Response Nursing program will help to bridge the transition from acute to community care. This session is accredited by the College of Family Physicians of Canada and the Ontario Chapter for 2.25 Mainpro‐M1 credits. For more information, please click here.

Self Management Training Opportunities for Staff

This program is designed to acquaint the participant with the literature, theory and techniques for promoting positive change in health behaviour. Professionals and staff from all different disciplines have attended these workshops, and have given great feedback. The workshop is designed for small groups of 18-22 participants and consists of mini-lectures, interactive exercises, videotaped case studies and skill practice with peers. Workshop delivery and all materials will be provided FREE OF COST to your organization.
